Obituary For Patricia Ann Irwin
Patricia Ann Irwin, 87, entered into eternal rest on June 21, 2026.
A native of New York and longtime resident of Chesapeake, Patricia devoted her professional life to caring for others as a nurse practitioner. Her compassion extended beyond her career, touching the lives of family, friends, neighbors, and all who had the pleasure of knowing her.
Patricia was preceded in death by her beloved parents, Mary and John Irwin.
She leaves behind both her beloved daughter, Ariane Fantin, who cared for her every day on Hospice; and her favorite grandson, Ian Segarra, who will forever cherish her love and memory.
A life devoted to serving others leaves behind a remarkable legacy. Patricia’s kindness, wisdom, and gentle spirit will continue to be remembered with deep love by those whose lives she enriched.
At the family’s request, arrangements are being handled with care by Simply Cremation.

My +Aunt Pat was a unique and beautiful woman. She was kind and generous. I only wish I had lived closer to have spent more time with her. I did spend a few days with her and my cousin Ariane in 1994 sharing our Catholic faith together. May +Aunt Pat rest in Christ's eternal peace and may the Angels carry her to the heavenly mansions. Ms. Pat was a once in a lifetime gift from heaven. She has remained in my heart and thoughts for these last 35 years, and will continue to inspire me. She took me in and redirected my path, shared forgiveness and love and inspired a brighter future that I never would have known without her. I am grateful for the incredible gift she gave me. Thank you, Ms. Pat and I am deeply sorry for her family. My prayers are with you.

Ariana and Ian, I am so sorry for your loss. I worked with your mom for 24 years at PHD. She was a kind a caring practitioner and a wonderful co- worker.
